Addictions Case Manager

A dynamic opportunity to help individuals with substance use and mental health disorders in collaboration with agency and community providers. Primary responsibilities for the case management of 35 individuals with substance use and mental health disorder.

Full-time / Non-Exempt

Pay range begins at $20.00

QUALIFICATIONS

Bachelor’s degree in Social Work, Psychology or related field (candidates with an Associate's degree plus a minimum of 3 years’ experience will be considered). CCB certification a plus. Must have proven clinical, verbal and written communication skills. Must be able to work independently and in the community, representing the values and philosophy of McCall in all interactions. Ability to speak Spanish preferred.

 Physical Requirements: Prolonged periods of time sitting at a desk, using the phone, and working on a computer. Must be able to lift up to 20 pounds at times.

Valid Driver’s license required.

KEY RESPONSIBILITIES

  • Engage people in the community in their recovery journey by treating each person as a resource and the expert in their needs and next steps.
  • Develop service plans with clients by identifying and ranking problems needing resolution; establishing reasonable and achievable goals; determining a recovery plan and resources to be used; and initiating discharge planning and monitor recovery plan progress.
  • Advocate for clients in various systems to assure quality treatment and equity of care.
  • Provide crisis prevention and intervention by identifying and helping clients to identify triggers for and/or symptoms of relapse, and early intervention to prevent further setbacks.
  • Enters documentation into electronic health record.
  • Performs other duties as may be assigned by Director of Special Services.
